Your First Visit

Thank you for choosing Dr. Sameer Mathur as your orthopaedic provider of choice. We look forward to getting to know you at your first appointment. In order to expedite the check-in process, we ask that you please fill out your new patient paperwork prior to your appointment. If you are unable to fill out your paperwork beforehand, please plan to arrive 15-30 minutes prior to your appointment in order to properly allocate time to fill out all documents.

Parking

All of our offices offer on-site parking. If you need assistance getting to and from your vehicle, please contact our office to make arrangements. We have wheelchairs and staff members available for patients in need.

Check-In

We ask that all patients arrive at least 15 minutes prior to their appointment time. This will give our staff enough time to collect insurance information, contact information, and any additional items that may be needed, such as a copay. We value the time of our patients and strive to keep wait times as short as possible. For this reason, if you arrive late to your appointment, it is possible that we may have to reschedule.

Please plan to bring the following items with you to your appointment:

  • Government issued photo ID
  • Insurance card
  • Copies of operation records, medical records, X-rays, MRI’s, CT scans, bone scans, and ultrasounds
  • If you have seen a physical therapist, please bring a progress letter from the therapist.
  • If you have had surgery elsewhere, please bring a copy of your operation report if that is available.
  • A list of all medications that you are currently taking and their doses.
  • A list of any known drug allergies and the symptoms you may have from taking these medicines.
  • Your health insurance information
  • Your claim number if it is a workers compensation injury

X-Rays

In certain cases, Dr. Mathur may request an X-Ray to aid in your spine pain treatment. Each of our offices is equipped with X-Ray machines and offers onsite imaging. We ask that patients who believe an x-ray is warranted dress appropriately for their appointment or bring a change of clothing that allows that body part to be accessed. Gowns or clothing will be provided as needed.

Your Appointment

After the check in process has been completed, you will be shown to your exam room. At this time. Dr. Mathur will provide you with an evaluation. Dr. Mathur is dedicated to providing patients with the highest level of care and spends adequate time getting to know each patient in order to determine an accurate diagnosis and the best course of spine pain treatment. Please be prepared to answer questions in regards to your symptoms, pain level, and medical history. You can rest assured that during your appointment you will receive Dr. Mathur’s undivided attention.

Check Out

We ask that you check out at the counter following your appointment. At this time, if you have questions regarding consultation or surgical fees, our staff will gladly assist you. Additionally, you may schedule your next appointment at this time. Once again, if you need assistance getting to your vehicle, our staff will gladly assist you.
